CHPD R&D website prize draw terms and conditions

The competition is only open to persons who:
a. are aged 18 or over;
b. have registered their details on the CHPD R&D website
c. have completed and submitted all questionnaires contained in the stipulated CHPD research study
d. have a valid email address for notification purposes.
2. No purchase is necessary
3. The competition is not open to employees or staff of CHPD, or their agents, families or partners.
4. No more than one entry per person.
5. The prizes are three lots of £100 Amazon vouchers. There will be three winners, each of whom wins one lot of £100 vouchers. There is no cash or prize alternative.
6. There will be three prize draws; one in January 2008, one in February 2008 and one in March 2008.
7. Winning entries will be selected at random from those submitted through the CHPD research portal by 5pm on 17 January 2008 for the first draw, by 15 February 2009 for the second draw and by 14 March for the third draw. Each draw for a winner will take place within two weeks of these dates. Previous winners will be excluded from later draws.
8. No late, delayed or lost entries will be considered. Proof of entry is not proof of receipt.
9. Winners will be notified by email to the email address they supplied with their entry. If they do not claim their prize within one month of notification, they forfeit their prize and another random winner will be selected in their place.
10. Prizes will be sent by post and no responsibility is accepted for lost or damaged prizes.
11. The names and location of winners will be available on request after 31 March 2008 by emailing info@chpd.com

13. All entrants must accept these rules. In the event of any dispute about the rules, the dispute will be passed to the chief executive of CHPD, and her decision will be final.
